Individualized Family Service Plan

What is an  IFSP?

An IFSP is a flexible document that addresses the needs of both your child and your family. Developed by you and your PIC early intervention team, it assures that your family's goals and needs are recognized in a way that respects its values and beliefs. The IFSP reflects your family's priorities for your child and family, and also identifies your resources.

A valuable tool for you, your child, and your early intervention provider, the IFSP serves as a guide to indicate your child's therapy and intervention goals, how they are progressing, and who is responsible for carrying them out. An IFSP must be updated at least every six months and rewritten annually. However, it may be reviewed and updated as necessary throughout the year. Reasons for updating the IFSP may include stating progress toward goals, modifying frequency of services, adding or deleting goals, or beginning transition.

The completed IFSP will include the following information:

  • Family information, such as resources, priorities, and concerns;
  • Goals for the child and family, and who will assist with achieving these outcomes;
  • Statement of services, including frequency of visits, time, location, provider names, cost, and payer;
  • Strategies that will be used to achieve goals;
  • Information regarding child's physical, cognitive, social-emotional, communication, and adaptive development;
  • Signatures of everyone participating in writing, updating, or changing the IFSP.
